Liability Waiver and Release Form

I, the undersigned participant (or parent/guardian of a minor participant), voluntarily choose to participate in wrestling activities, training sessions, competitions, and related events organized by No Mercy School of Wrestling (hereinafter referred to as “the Organization”).

1. Acknowledgment of Risk

I acknowledge that wrestling and related physical activities involve inherent risks of physical injury, including but not limited to:

  • Sprains, strains, fractures, and dislocations
  • Concussions and other head injuries
  • Muscle and ligament tears
  • Skin infections (e.g., ringworm, staph infections, herpes simplex)
  • Communicable diseases, including but not limited to COVID-19, influenza, and other transmissible illnesses
  • Bruises, cuts, and abrasions
  • Heat-related illnesses
  • In rare cases, catastrophic injury, paralysis, or death
2. Assumption of Risk

I fully understand and voluntarily assume all risks associated with participation in wrestling activities, whether or not described above. I understand that these risks may result from the actions, inactions, or negligence of myself, other participants, coaches, officials, or third parties.

3. Waiver and Release of Liability

In consideration of being allowed to participate, I, on behalf of myself (and my minor child, if applicable), my heirs, assigns, and personal representatives, hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE No Mercy School of Wrestling, its owners, coaches, volunteers, officers, employees, agents, and affiliated organizations from any and all liability, claims, demands, and causes of action — including those arising from ordinary negligence — arising out of or related to any loss, damage, or injury, including death, that may be sustained during participation. I understand that this release does not apply to gross negligence or willful misconduct.

4. Medical Authorization

In the event of injury during participation, I authorize No Mercy School of Wrestling and its representatives to seek and obtain emergency medical treatment for me (or my minor child). I agree to bear all costs of such medical treatment. I confirm that I am the legal parent or guardian of the minor participant named in this form, and that I have full authority to execute this authorization.

5. Media Release

Media Consent is optional and separate from the waiver above. By initialing below, I grant No Mercy School of Wrestling the right to use photographs, video recordings, and other media of the participant without compensation. Media may be used on the following platforms:

  • No Mercy Wrestling Instagram account and Instagram Stories
  • No Mercy Wrestling website (nomercywc.com)
  • YouTube (match footage, highlights, and promotional videos)

6. Rules and Code of Conduct

I agree to abide by all rules, regulations, and codes of conduct established by No Mercy School of Wrestling. I understand that failure to comply may result in removal from the program. In such cases, fees paid for the current calendar month are non-refundable. Fees paid for future months beyond the current month will be refunded on a pro-rated basis at the sole discretion of the Organization.

7. USA Wrestling Membership Requirement

A current USA Wrestling Athlete Membership card is required for all participants and should be obtained as soon as possible. Specifically:

  • Tournament / Competition: A valid USA Wrestling card must be obtained before competing in any sanctioned tournament or competition. No exceptions will be made — you will not be allowed to compete without it.
  • Practice: New participants are granted a 30-day grace period from their first practice to obtain a USA Wrestling card. After 30 days, a valid card is required to continue attending practices. Do not wait — get your card immediately.

USA Wrestling membership includes secondary sports accident insurance covering practices and competitions. Participants who have not yet obtained their card during the grace period acknowledge that this insurance coverage will not apply to them. Membership can be purchased at usawmembership.com. The membership year runs September 1 through August 31 and must be renewed annually.

8. Governing Law

This waiver shall be governed by and construed in accordance with the laws of the State of California. Any disputes arising under this agreement shall be resolved in the courts of San Diego County, California.

9. Severability

If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.

10. Acknowledgment

I have read this Liability Waiver and Release Form, fully understand its terms, and had the opportunity to ask questions before signing. I sign this document freely and voluntarily without any inducement.
